Orbital cellulitis

Definition:-

Orbital cellulitis is acute suppurative inflammation of the orbital cellular fibro_fatty tissue.

Etiology:-

Orbital cellulitis is caused by direct infection from penetrating wounds or following trauma/spread of infection from nose, teeth, globe/blood borne as in septicemia.

Orbital cellulitis is caused usually by staphylococcus, streptococcus, pneumococcus and rarely fungi.

Clinical picture:-

Symptoms:-

  1. Fever, malaise.
  2. Local pain.
  3. Vision is good at the beginning , but latter it decreases due to optic neuritis.

Signs:-

  1. Edema of lid and conjunctiva.
  2. Proptosis.
  3. Limitation of ocular motility.
  4. Formation of an abscess.

Complications:-

  1. Brain abscess and cavernous sinus thrombosis.
  2. Optic neuritis.
  3. Endophthalmitis.
  4. Central retinal vein thrombosis.
  5. Frozen orbit.
  6. Corneal exposure.
  7. General spread leading to septicemia and pyemia.

Treatment:-

  1. Antibiotics.
  2. Hot formentation.
  3. Incision and drainage if abscess is formed.

Treatment of corneal exposure.
